A new study suggests that aripiprazole, a widely used antidepressant, may help patients with bipolar disorder with manic episodes or maintenance mania.
Bipolar disorder is a mood disorder in which patients are moody and have trouble focusing or concentrating, as well as being irritable or angry. They may also have trouble sleeping or appetite control.
This is the first time the FDA has approved an antidepressant for use in pediatric patients, the researchers say.
But the new study, published in the, and the results of two other studies, could prove useful for physicians treating bipolar disorder. The first was a randomized, placebo-controlled trial of 15 patients, all children and adults, who had bipolar disorder and had the same symptoms.
The researchers enrolled 15 children who were treated with quetiapine (the generic name for the brand name Seroquel) in an open-label trial. Those with manic symptoms or bipolar changes were assigned to receive either 15 mg/day of quetiapine for 6 months or 10 mg/day for a year, as appropriate. The treatment group received a placebo. The researchers also treated patients who had not yet been diagnosed with bipolar disorder, or who were taking lithium or valproate. The participants were asked to take the medication at bedtime for 6 months. The study participants were given an antidepressant pill or a placebo for 7 weeks, then were asked to continue taking the medication for the next 7 weeks, until the next day of treatment with quetiapine or placebo.
The researchers concluded: “In this study, patients who had been taking the quetiapine-based combination for 6 months and then had bipolar changes and had not been on a manic treatment medication for more than 7 days showed improvement in clinical symptoms.”
The researchers also recommended that patients take the medication for 7 days after the onset of symptoms. Patients should continue taking the medication for the next 7 days. They also recommended that patients have a complete medical history including depression, anxiety, suicidal thoughts, and personality disorders. They also recommended that they have a physical examination to rule out other psychiatric conditions, and that patients not use drugs for a long period of time.
The researchers found that the group who took the antidepressant dose of quetiapine had a significant improvement in symptoms of bipolar disorder, compared with the group who took placebo. But the researchers also recommended that patients have a complete medical history including depression, anxiety, suicidal thoughts, and personality disorders.
“While this study supports the effectiveness of the antidepressant quetiapine in treating bipolar disorder, it does not support the use of this drug in patients with bipolar disorder,” the researchers said in a statement.
The FDA approved the drug for pediatric use in 2002, and quetiapine is approved in pediatric use in both adults and children. It has been approved in the US since 2007.
The study is the first in a new class of drugs called atypical antipsychotics, which are medicines that are used to treat mental illness and other symptoms of mania, agitation, anxiety, depression, psychosis, bipolar disorder, and other mental health conditions.
While it is not officially known which drug is the most effective, doctors usually prescribe it to patients with bipolar disorder and schizophrenia, as well as to patients with bipolar disorder and major depressive disorder.
The research was funded by Lilly and Company. Its publication was published online in theNew England Journal of Medicine.
